What went wrong with my CNC Lathe set up?

So I did 9 setups today and crashed 2 center drills. I have know Idea what is wrong with my set-up. The program starts. There is a M00 for indicating. I indicate. Next line there is a M00 for work shift. I make sure tool 4 offset 4 is up. I touch off my workshift, with the .100 block. I enter “offset measure” then MZ.100 The off set moved a little bit more then the last set up. So I enter in again. Numbers did not change. I try it again check that the tool and offset are up. Nothing has changed. I creep up on the part and it says distance to go 3.0 it looks like 3.0 its actually more but I cant tell by eye. I get close enough and let it run and Bam work shift was off one time by .050 and the 2nd time almost 2in. What did I do wrong if I know I set it correctly and am confident that I did.

Best bet is to ask senior man in the shop with the most experience on the machine. Off hand it sounds like the machine did not know where it was when started or your position sensor hardware or software is messed up. The jump on start up can be mechanical vibration messing up a contact or a loose mechanical part someplace. Until you can get it figured out, please tag the machine. People have been known to be hurt by broken bits.
Hint. Has anyone taken the machine apart recently and gotten cutting oil into a connector or into the circuit boards?
